Should there be some type of plate/cover over this? It's my first v drive so just wondering. I was cleaning under the boat and noticed it.

No. But that driveshaft has been replaced. I'm betting the previous owner hit something and took a few chunks out of the hull that you can see.

Loose bolt lower left?

Looks normal to me. Bolts holding the log in may be loose as bftskir pointed out, check those and if loose I'd dap 5200 under the heads.. You can sand and either gel patch or smooth some 5200 over those small ships around the hole. Align it if it hasn't been done in a few years.
